Admission Requirement

Age Requirement
Must be 18 years or older
Ability to Live in a Shared Setting independently
Must be able to live safely with others in a shared housing environment
No active violent behaviors or history that would endanger other residents
Medical & Behavioral Stability
Must be medically stable (no acute hospitalization needs)
Documentation Required Valid ID (state-issued ID. driver’s license, or VA card)

Proof of income or benefits (SSI. SSDI, VA benefits. Medicaid, etc.)
Current TB test results and proof of immunizations if applicable
Background check (to ensure safety of residents)
Commitment to House Rules
Must agree to follow house rules, curfew, and participation in meal/activity
Willingness to engage in supportive services (transportation, appointments, case mangement).
